All services require a referral for OHIP coverage to apply.
Please have your physician, nurse practitioner or midwife complete this HealthCare Provider Referral Form and submit it Online or by Fax.
To book your visit, simply click on our BOOK NOW buttons found on the top of this page. We will ask you a few questions to get started and then our lovely admin will send you an email shortly after with your intake form.
You'll also get a few text messages to help us walk through getting you booked in as soon as possible.
